Why $2M Is Easier Than $1.4M (And What to Focus on Next)

I recently spoke with a doctor who asked for help increasing revenue from $1.2M to $1.4M. And interestingly, that sparked a realization for me, too.

 

Here’s the counterintuitive truth:

👉 It’s often easier to think about going from $1.2M to $2M than from $1.2M to $1.4M.

Why? Because small jumps keep us stuck in incremental thinking. Big jumps force leverage.

 

 

The Math (Quickly)

If one doctor is producing $1.2M, that’s roughly:

  • ~$100K per month
  • ~20 patients/day
  • ~$200 per visit

To reach $2M, you need an additional $800K/year, or about $67K per month.

That doesn’t come from hustle. It comes from structure, belief, and leverage.

 

 

The #1 Lever: Increase Revenue Per Patient

The lowest-stress, highest-impact move is raising your visit value.

If your average visit goes from:

  • $200 → $300 (or even $400)

You don’t need more patients—you need better conversations, better structure, and better follow-through.

 

 

What I’m Personally Working On

I’ll be honest—this is as much for me as it is for you.

1. Treatment sheets (used consistently)

I’m great on visit one…but less consistent on visit two.

Treatment sheets keep the care structured and prevent leakage.

 

2. Phase 1 / Phase 2 care
  • Phase 1: insurance-based, early relief
  • Phase 2: higher-value solutions when Phase 1 isn’t enough

This gives you a natural reason to revisit options instead of “selling.”

 

3. Conviction matters

I can sell shockwave all day—because I believe in it.

Laser for nail fungus? I struggle…because my belief isn’t strong yet.

Patients feel that.

Sometimes belief comes before volume. Sometimes volume creates belief.

 

4. Bundling & standard of care

What if the standard wasn’t just shockwave—but:

  • Shockwave
  • Orthotics
  • Amnio

Instead of $1,500…now you’re at $3,600.

That shift alone can change everything.

 

 

The Hidden Money: Recall & Referrals

Two big gaps I see in my own practice:

  • Recall systems (patients slipping through the cracks)
  • Referral source nurturing (no consistent lunch & learns, touchpoints, or follow-up)

I still have open slots—at $1.2M.

Filling those consistently could push the practice toward $1.5M–$2M without more marketing.

 

 

Buying Your Time Back

This part is working well for me:

  • Scribes
  • Staff-led education
  • Shorter follow-ups
  • Pre-built explanation sheets

The next step is being less apologetic about asking my team to help me scale.

 

 

Bottom Line

Getting to $2M isn’t about:

  • More hustle
  • More hours
  • More chaos

It’s about:

  • Higher visit value
  • Stronger belief
  • Better systems
  • Fewer leaks
